We are seeking a compassionate and dedicated
Social Worker (LMSW or LCSW) to join our team in Seneca, NY. This role is part
of a Program of All-Inclusive Care for the Elderly (PACE). The Social Worker
will play a key role in improving the quality of life for participants through
advocacy, assessment, planning, and direct support in various aspects of their
lives, including psychological, psychosocial, financial, environmental, and
interpersonal matters.
This position requires collaboration with
participants, their families, interdisciplinary care teams, and community
partners to ensure a seamless continuum of care.
Compensation: $40-$50
Per Hour
Schedule:
- Monday – Friday, 9:00 AM – 5:00 PM
- Full-time, on-site and fieldwork as needed
Key Responsibilities:
- Advocate for participants and their families
to ensure they receive comprehensive, compassionate care.
- Assess and evaluate psychosocial
functioning, including conducting mental status assessments.
- Assist participants with navigating complex
issues such as adjustment to illness/disability/treatment, financial
concerns, and environmental challenges.
- Provide crisis intervention, end-of-life
care, and emotional support to participants and their families.
- Collaborate with the Interdisciplinary Care
Team (IDT) and community partners such as hospitals, skilled nursing
facilities (SNFs), assisted living facilities (ALFs), and Adult Protective
Services (APS).
- Participate in discharge planning and care
coordination to ensure smooth transitions between care settings.
- Enlist and connect participants to community
resources and agencies to address unmet needs.
- Deliver education to participants and their
caregivers to promote health, wellness, and independence.
- Conduct home visits, visits to PACE sites,
and travel to community settings as required.
Requirements:
- Master of Social Work (MSW) from an
accredited college or university
- Active New York State LMSW or LCSW license
- Minimum of one (1) year experience in a
healthcare setting – required.
- One (1) year experience working with a frail
or elderly population preferred.
- If under one year of experience, must
complete PACE training upon hire.
- Experience in home care and/or field work
preferred.
- Regular access to a car for field visits preferred, but not required.
