Temporary to Perm Nurse

We are seeking a Temporary to Perm Nurse to deliver high-quality, compassionate nursing care to patients in their homes in Bronx, NY.  The ideal candidate demonstrates strong clinical judgment, independence, and a patient-centered approach to care while working across a designated service area.

Compensation:   $50-58+ Depending on experience plus tolls and gas reimbursement

Schedule & Work Requirements:

• This is a 100% field-based position with Temp-to-Perm potential.
• Five (5) days per week, even when weekend coverage is required.
• Weekend coverage required 1–2 weekends per month as part of a rotating schedule.
• When scheduled for a weekend, the nurse will receive one weekday off prior to the Saturday shift and one weekday off the following week after the Sunday shift.
• Valid driver’s license and reliable personal vehicle required for travel within the assigned service area.

Responsibilities:

• Provide routine home visits for wound care, medication administration, and health assessments.
• Deliver care per individualized care plans, physician orders, and nursing standards.
• Monitor patient conditions and communicate changes to the care team.
• Educate patients and caregivers on disease management, medications, wound care, and home safety.
• Document care accurately in the electronic health record.
• Collaborate with interdisciplinary teams and partner organizations.
• Provide nursing support in a Diagnostic & Treatment Center or medical practice as needed.
• Travel within the assigned service area and comply with all clinical, regulatory, and infection control standards.

Qualifications:

• Degree from an NLN-accredited nursing school.
• BSN required, or RN with a minimum of five (5) years of healthcare experience.

• Current, active, and unrestricted Registered Nurse (RN) license in the State of New York.

• Minimum of three (3) years of clinical nursing experience required.
• One to two (1–2) years of home care or similar healthcare setting experience preferred.
