A healthcare organization in Brooklyn, NY is seeking a Temporary Clinical Assessment Manager to support clinical intake, enrollment, and reassessment operations within a managed care environment. This opportunity is ideal for a Registered Nurse with experience in home care, MLTC, managed care, or community-based assessments who enjoys working independently and supporting care coordination for adult and geriatric populations.

The Clinical Assessment RN will assist with completion of UAS-NY assessments, reassessments, and related clinical documentation to support quality patient care and regulatory compliance.

 

Compensation:  $70-$80 Per Hour

 

Schedule

  • Monday through Friday
  • 8:30 AM – 5:30 PM

 

Key Responsibilities

  • Conduct in-home and/or virtual clinical assessments for program participants
  • Complete UAS-NY assessments and reassessments in a timely and accurate manner
  • Perform clinical onboarding evaluations and post-discharge follow-up assessments
  • Review medications, diagnoses, and clinical documentation for accuracy and completeness
  • Document assessments and participant information within the electronic medical record system
  • Collaborate with Intake, Enrollment, and Care Coordination teams to support continuity of care
  • Assist with identifying changes in condition and communicate findings appropriately
  • Ensure compliance with New York State Department of Health guidelines and organizational policies
  • Maintain timely completion of required documentation and assessments 

 

Qualifications

  • Active New York State Registered Nurse license required
  • Bachelor of Science in Nursing (BSN) or other health-related degree preferred
  • Minimum 2+ years of nursing experience in one or more of the following:
    • Managed care
    • MLTC
    • Home care
    • Care management
    • Community health
    • Utilization review
    • Discharge planning
  • Experience completing UAS-NY assessments preferred
  • Experience working with adult or geriatric populations preferred
  • Strong documentation and organizational skills
  • Comfortable working independently in the field and remotely
  • Proficiency with EMR systems and Microsoft Office
  • Reliable transportation and willingness to travel locally required
